• Initiating, developing, implementing, and revising the plan of care in collaboration with the physician and other healthcare professionals.
• Supervising care provided by home health aides and licensed practical vocational nurses, providing instruction as necessary, and assigning tasks according to State and federal regulations.
• Making initial and comprehensive nursing evaluation visits, ensuring patients meet home health eligibility and medical necessity guidelines, and developing the plan of care within State-specific guidelines with the physician.
• Communicating relevant information timely and effectively with agency staff, including patient care issues or needs, visit assignments, and schedule changes.Required qualifications include:
  • Current RN licensure in the state of Texas.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle.
