Job Description:

Care Transition Coordinator- RN with Hospice Experience:

We are seeking a skilled RN to facilitate seamless transitions for patients discharging from facility settings to LHC Group hospice agency care.

Key responsibilities include:

--- Developing trusting relationships with patients and families as an end-of-life care expert

--- Valuing and respecting patients and their families

--- Providing employee-focused wellness and support programs

--- Offering incredible team support and empathetic leadership

--- Facilitating a smooth transition for patients from facility settings to hospice agency care

--- Assisting with the preparation for accepting care of patients post-discharge from hospitals

--- Explaining hospice services and agency procedures to patients and their family members

--- Involving families and caregivers in educational processes and assessing post-discharge coaching needs

Requirements:

--- One year of hospice experience or one year of hospital case management experience

--- Current Registered Nurse (RN) licensure in the state of practice

--- Excellent organizational and verbal/written communication skills

--- Thorough understanding of hospice qualifying criteria and coverage guidelines

--- Proficient computer skills

--- Current CPR, driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation
